• In addition to this form, your parent or legal guardian needs to fill out the Parental Consent Form (which can be downloaded below).
• Please return all the Minor Assent Form and Parental Consent Form to the Student Services Office (Jordan Hall, Building 420, Room 422). Forms are due Friday, the second week of class.
